ICS-43434 overheating and dying on new production batch

Hello: We just completed a second production batch of our product using the ICS-43434 after a previous successful run. On this batch, the ICS-43434 overheats at power up and dies quickly. Is there a known batch issue? The lot code is 434-VQG6J
